I give permission for my youth to attend YOUTH SUMMER FUN.  I acknowledge that accidents happen that may cause personal injury to my youth.  In case of an emergency, I understand that the Youth Ministry personnel will make every effort to contact me.  I hereby authorize Resurrection Life Church to seek and consent to emergency medical attention for my youth as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention. 

I hereby release Resurrection Life Church, its employees, agents and volunteers, from any and all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage or injury that may be sustained by my child while participating in or traveling to and from this event. 

The act of submitting this form is intended to and will constitute a legally binding signature and will create a legally binding agreement.
